Just loosen the hinge bolts where they attach to the door and use a hydraulic floor jack to lift the door to where you need it. There's not a lot of room there for upward travel, but between those bolts and the hinge-to-cab bolts, you should be able to get enough. I'd imagine when you installed the door the bolts were probably tightened up with the bolts towards the bottom of each hole, due to the weight of the door. Use a block of wood between the jack and door to lift it back up and you should be OK. It'll probably take a little trial-and-error to find the right spot, though.
